2024-25 Club Season Checklist

Step 1
Age Eligibility: 
Check with the AZ Region to verify the correct age group for players. 

Step 2
Create a SportsEngine Account:
Please click the link above if you do not have a SportsEngine account. 

Step 3
Purchase Annual AZ Region Membership:
All players must register with the Arizona Region to participate in all
USA Volleyball related activity, including tryouts, regular season, and post-season.
Please click the link above for membership registration.

Step 4
SafeSports Education Waiver:
The AZ Region requires all parents and players to read through the
SafeSports three-page waiver.
Please have only the last page of the waiver signed and ready to hand in at tryouts.

Step 5
Concussion Waiver:
The AZ Region requires all parents and players to read through 
the Concussion waiver. Parents and players must acknowledge understanding 
of symptoms along with the proper response of what to do in case of 
a concussion. Please have the waiver signed and ready to hand in at tryouts. 

Step 6 
Medical Release Waiver: 
Both parents and players must sign this form. 
Please have the waiver signed and prepared to hand in at tryouts. 

Step 7
14U Open Gyms:

18U Open Gyms:
Please come out and meet the Amazons, and let’s have some fun. 

Step 8
Birth Certificate:
Please bring a copy (not the original) of your child's birth certificate
to tryouts If you are a first-time member of the AZ Region of USA Volleyball.

14U Tryouts:

18U Tryouts:
Please have all the necessary paperwork with you, along with a photocopy 
of your AZ Region membership.
Entry into tryouts will not be granted without an AZ Region membership.
